I had a smiliar thing too with the scan I had at 13 weeks. After waiting an hour and 40 minutes - someone didnt turn up for work that day! - we finally went in but it seemed rushed and generally wasnt a relaxed comforting scan like I'd had with my previous children. But we saw baby on screen and everything seemed ok so we went home, told the children they were having a brother or sister and started to make plans and got attached to the new baby.
Of course at 21 weeks that all changed as the scanner realised Adam's skull hadnt formed properly and like you I wanted to know why something so major wasnt seen before. I was told it was to do with the position he was in, they couldnt see his head clearly. Which is a rubbish answer really because if they couldnt see, they should've asked me to come back and checked.
Nothing would have changed the outcome - Adam was still going to die no matter what - but I was so angry for ages that theyd let me get attached to this little person (having had a mmc before I basically waited for the scan to make sure this baby was alive and well).
A lot went wrong that day when I had the 13 week scan - just hope no other mother later received bad news because they werent up to their job.
